Healthy Advantage is a Medicare Advantage Prescription Drug Special Needs Plan for people with Medicare and full Medicaid. You are eligible for Healthy Advantage* if you:
- Are entitled to Medicare Part A
- Are enrolled in Medicare Part B
- Are entitled to full Medicaid
- Reside in the Healthy Advantage service area

- Enrollment form - Please complete this form to enroll in the plan. If you need help in completing the form, please contact Molina Medicare at 1-866-403-8293 (TDD/TTY: 1-800-346-4128) from 8 a.m. to 8 p.m. Monday – Sunday. You may enroll in Healthy Advantage at any time during the year.
- Evidence of Coverage (EOC) - The EOC explains your rights, benefits, and responsibilities as a member of Healthy Advantage. It also explains our responsibilities to you.
- Summary of Benefits / Directories - The Summary of Benefits outlines your plan premium, benefits, copayments and coinsurance for the current calendar year. Healthy Advantage offers a broad provider network across the Wasatch Front.
- Low Income Subsidy (LIS) Chart - This table shows you what your monthly plan premium will be if you get extra help from Medicare in paying certain prescription drug costs.
- Forms – The following forms may be helpful and are available through this link: Appointment of Representative Form (CMS-1696), Coverage Determination Request Form, Drug Determination Request Form, How to Request a Re-Determination?, and Pharmacy Direct Member Reimbursement Form.
- HIPAA, which stands for the American Health Insurance Portability and Accountability Act of 1996, is a set of rules to be followed by doctors, hospitals and other health care providers.
- Notice of Privacy Practice - This notice describes how medical information about you may be used and disclosed. It also explains how you can get access to this information.
- Contact HIPAA - This link provides information on how to contact the HIPAA office in your state.
- Over-the-Counter (OTC) (2012) – This is a benefit offered by your plan that can be used many different ways. We are partnered with CVS Caremark so that you can order the health and wellness products you need most. You can order online or place an order by telephone. Please use the link to view the catalog of items available as well as an explanation of how to place your order.
- 24 Hour Nurse Advice Line for Healthy Advantage members: 1-866-472-0601 (TTY: 1-866-735-2929) There may be times when you may need care and your Primary Care Provider (PCP) is closed. If it is after hours and your PCP’s office is closed you can call your PCP or Healthy Advantage’s Nurse Advice Line. Nurses are available to help you at any time of the day. Call 1-866-472-0601, 24 hours a day, 7 days a week.
- Medical Transportation for Healthy Advantage members: If you need a ride to your health care appointments, we can help - this includes things like PCP office visits, labs, x-rays or therapy. Non-emergency transportation services do not require prior approval.
We can schedule transportation on a recurring basis ahead of time. If you have healthcare-related transportation needs, please call the contact info below, as we can arrange a wheelchair van or other services if needed. Please call LogistiCare at (877) 564-0577 for reservations and (877) 564-0578 for Ride Assist. TTY/TDD users please call 711. Please note it is important to call 3 days in advance of your appointment to schedule the transportation.
