Healthy Advantage (HMO SNP) is offered in the following counties: Davis, Salt Lake, Utah and Weber.

Molina Medicare Options Plus (HMO SNP) is a Medicare Advantage Prescription Drug Special Needs Plan for people with both Medicare and Medicaid. It offers all the benefits of Original Medicare and more.
